The Approved Workman Cloud database has the ability to customize text for common forms that are signed by parents or leaders consenting to the terms of the church/club for a Medical Release, Activity Participation, Photo Use and Travel. Additional custom forms may be added as needed. Each consent form can be associated with either clubbers, leaders, or both to handle different requirements between clubbers and leaders. Consent Forms are stored by club year, so forms can be updated for each club year without losing historical data.


The Consent Forms are managed within the Club module. Under Setup, select the Consent Forms page. The Consent Forms can also be handled within the Registration module on the Manage Site page.


Tracking the acceptance of the Consent Form is handled on the Members > Personal page. Acceptance may be indicated by checking the "Accepted" box for a specific form when the page is in Edit mode. For forms used in Online Registration, the box will be checked when a registration with a signed consent is accepted. The signature and date information from the registration will be shown. Consent Form data collected through the Online Registration site will be shown on the edit page but cannot be changed.


To create a new consent form for the current club year, tap/click the Add Consent Form buttonto open the Add Consent From dialog. Complete the following information:

  1. Consent Type (required) - create a name for the consent form that describes its purpose.
  2. Consent Text (required) - enter the text to be used on the form.
  3. Visible in Registration - fields with this box checked will be shown in the Online Registration site.
  4. Required - if the form is visible in registration, check the box if the field is required to be completed.
  5. Member Type - select who needs to complete the form (Clubbers, Leaders, or Both)


Multiple custom Consent Forms may be added.


To make changes to a form, select the form and tap/click the Edit buttonEdit club info


Tap/click the Delete buttonDelete memberto remove a Consent Form.


WARNING: Do not delete a Consent Form that has related member data (signed/accepted consents). A specific form can be safely removed at the beginning of a new club year before receiving signed/accepted consents.
